Karin Jonczak at Abington Memorial Hospital made a wonderful suggestion to compile the post-discharge outcome assessment visit worksheets into packets. Nichol McBee kindly compiled these sets, one for Day-30, Day-90, Day-180, Day-270, and Day-365 as a zipped set of PDF forms that you can place on your desktop for quick printing.
Each set (or packet) has the Day-8 IVC removal form since the date for removal of the IVC is unspecified. The same consideration goes for Adverse Events, Conditions/Procedures, Death, Withdrawal of Care, Resource Utilization and Final Status forms.

On 28-February-2013, Henry Ford Heath System randomized the 327th patient in the CLEAR-III trial. In September-2012, the CLEAR-III team set the bar high with 17 enrollments. But, it took Just 5 months to beat that amazing record and set the bar even higher to 18 enrollments for the month of February.
